Select from one of the three Tournament Leagues offered. Note: Only one application per player will be accepted and should be completed by the account holder.

Use the drop down menu to select the desired Tournament Week.

Enter the account holder UB/AP name and first name. Note: UB/AP players account names and first names must match exactly to players accounts on UB/AP.

Check the appropriate designated box for the league.

Enter your email address. Double check your email address for accuracy as this is the only way we have to contact you.

Once you have completed the on-line application click Submit once.

Please allow up to 2 hours for a confirmation email that your league application has been received.
